- One application per household per death - Time frame of loss must be within two (2) weeks of request for support/assistance - Maximum limit of support/assistance $250.00 (will be provided in various resources) - Applicant and lost loved one must be a resident of Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, or Washington counties

Provides immediate assistance to parents who are experiencing the loss of a loved one from gun violence, including: - Connection to crisis services - Funeral clothing assistance - Relief food for the family - Spiritual support Funds are disbursed in the form of gift cards, with a maximum grant of $250 to eligible applicants
