Diabetes Prevention: a lifestyle change program for participants who are at high risk of getting Type 2 diabetes or are pre-diabetic. This one-year long program is developed by the Center for Disease Control and is evidence-based. The goal of the program is to help participants make small changes to lose 5 - 7% of their body weight and be active for 150 minutes of physical activity a week. Eligibility

Diabetes prevention: Individuals 18 years and older, with a body mass index (BMI) of 22 or higher, and who are pre-diabetic or high risk of developing Type-2 diabetes. Serves residents of Anoka, Carver, Chisago, Dakota, Hennepin, Isanti, Ramsey, Scott, and Washington counties.
