Office of Health Facility Complaints - Minnesota Department of Health

Serves the general public by investigating reports of health care facilities violating state or federal regulations. Addresses complaints, questions, or concerns related to licensed facilities: hospitals, nursing homes, boarding care homes, supervised living facilities, assisted living, and home health agencies. This office cannot investigate billing or insurance concerns, medical clinics, or personal care attendants who do not work for a home care agency. Though if an individual is unsure where to file their complaint, contact the office.
