MinnesotaCare - MinnesotaCare

MinnesotaCare is a health care program that is administered by the Department of Human Services and some counties. MinnesotaCare is available for people who do not have health insurance and meet income and household size guidelines. A monthly premium is generally required. Application process

Applications on MNSure website. Paper applications can also be found on the MN DHS website. Individuals can receive help filling out the application by contacting a navigator, calling 855-366-7873, in person at their local county office, or from the Senior or Disability LinkAge Line.

Fee

Monthly premium based on income, household size and number of household members covered. No application fee. Some recipients do not pay a premium.

Required documents

Completed Minnesota Health Care Application with verifications as applicable including: - Proof of income such as pay stubs or tax forms if self-employed - Proof of citizenship such a birth certificate or passport - Verification of immigration status such as Green Card - Verification of pregnancy signed by a doctor or nurse

Eligibility

To qualify, individuals must be a: - Minnesota resident - US citizen or lawfully present in the US - Meet the income limit - Not be enrolled or have access to Medicare Part A - Not be enrolled in Medicare Part B - Not be incarcerated, unless awaiting disposition of charges
