Program Fees
Provides mental health services which include: adult mental health case management, advocate for the needs of mental health clients, assist client in identifying mental health needs and referrals for services, completes functional assessment with case manager as well as individual community support plan, offers client outreach, coordinates with providers on resources relating to medication, housing, financial benefits, medical, social and vocational services. A case manager will monitor the quantity and quality of services provided to clients.
