Provides assistance and support with pregnancy care coordination to ensure a healthy delivery, including:
- Care Coordinator provides a needs assessment and continues to work with the mother
- Centering Program: Centering Pregnancy is available to program participants and is a type of prenatal care, where women with similar due dates meet as a group with a midwife
- Community referrals
- Providing after delivery care
