Program purpose is to support and empower seniors and caregivers by addressing their individual needs and preferences, enabling them to live safely and independently in their own homes, and communities, as long as possible - Assistance with MNsure - Confidential home visits - Eligibility determination and helping to complete forms for: - County benefits - Energy assistance - Medicaid and Medicare D - Home delivered meals - Home phone discounts and free cell phone program - Prescription drug discount programs - SNAP/EBT program - Help caregivers identify and meet needs of care receiver - Information and referral for: - Financial, legal and insurance issues - Health and safety - Housing - Long-term care consultation - Respite care and companionship - Ongoing contact and support - Training for caregivers
