- Call or walk in to a Human Services or Satellite Center location - Complete the Minnesota Health Care Programs Application for Certain Populations and return it to the county or tribal office. Application can also be mailed by calling the county or tribal office.

Medicare Savings Programs help people with low incomes pay the cost of their Medicare coverage. Medicare Savings Programs have four sub-programs with specific rules for qualifying: Qualified Medicare Beneficiaries: Helps pay all Medicare costs Service Limited Medicare Beneficiaries: Pays Part B premium only Qualified Working Disabled Individuals: Pays Part A premium only Qualified Individuals: Pays Part B premium only
