Children's Mental Health Services - Human Services of Faribault and Martin Counties
Assists and supports children diagnosed with a severe emotional disturbance and their families to gain access to needed medical, social, educational, vocational, and other necessary services as they relate to the child's mental health needs. Whether or not someone meets criteria is determined by a mental health professional in a diagnostic assessment. Children's mental health case management includes: - Assistance with accessing services: Helps the individual access needed medical, social, educational, vocational, and other services as they relate to the client's mental health, ensures coordination of services, and monitors the delivery of services. - Client outreach: Offers community support where the individual lives and provides face-to-face and phone contact in a non-intrusive manner. - Planning: Helps create life goals and plans by completing an assessment of strengths, needs, and the impact of the mental illness, and develops an individual community support plan to meet goals.
