Description
Program employs a multigenerational, trauma-informed approach in which Community Health Workers (CHWs) bridge gaps in health and social service systems. Through home visits and peer-based community gatherings, the program provides women with education, health system navigation, and social-emotional support to support reproductive health and early childhood development milestones. In partnership with health care providers, the program helps families access the right care at the right time while also strengthening cultural understanding and improving the effectiveness of clinicians in serving diverse communities.
