Description
Intensive adult outpatient services including group treatment and relapse/outpatient phase 4 services Provides case management services and referrals to individuals who are involved in outpatient or inpatient substance abuse services. Treatment Care Coordinators help individuals coming out of inpatient facilities to transition smoothly into aftercare services, housing, or other needed resources.
