- Call - All patients MUST call for an appointment. No clinic services available at the main office. - Patient will be asked income and health questions to determine eligibility. May be asked to provide prior medical records in some cases.

Primary medical care needs. Not able to provide dental, obstetrical, family planning, mental health, injury care including fractures, and other emergency type services. Clinics are located throughout the metro area with locations in: Minneapolis, St. Paul, Shakopee, Fridley, and Apple Valley.
