Goal is to help patients recover or manage health conditions in their own home, reducing the need for hospital or clinic visits. Services offered:- Home health aide support (help with daily living tasks, vital sign monitoring)- In-home monitoring / tele-health via remote devices (for chronic disease management like CHF, diabetes, hypertension, wound care)- Medical social services (support with emotional, social, and community-resource needs)- Palliative care for symptom management when needed- Physical, occupational, and speech therapy- Skilled nursing care
