NOTE: Funding is currently available for this program. Provides Family Homeless Prevention and Assistance Program - FHPAP. Services include: - Prevention Services: Temporary financial assistance and/or supportive services to help people remain in their current housing situation, relocate, or divert them from entering the homeless system. Financial assistance is provided for past due rent and/or for utility assistance to help individuals maintain or obtain housing. Limited case management is also provided. - Rapid-Rehousing: Assists households to quickly exit homelessness and return to permanent housing. Provides financial assistance such as rent, security deposit, and/or application fees to move into permanent housing and provides supportive services such as case management and housing navigation.

Eligibility

- Single adults, families, and youth who are residents of Burnsville, Eagan, Farmington, Lakeville, and Rosemount in Dakota County and are at imminent risk of homelessness or are currently homeless - Income at or below 200% of the Federal Poverty Guidelines
