The assessment takes a person-centered approach to ensure services meet each individual's strengths, goals, preferences, and assessed needs. Determines eligibility for publicly funded programs and services, including: - Alternative Care Program - Brain Injury Waiver - Community Alternative Care Waiver - Community Alternatives for Disabled Individuals Waiver - Developmental Disability Waiver - Elderly Waiver - Case management for people with developmental disabilities - Semi-Independent Living Services

Minnesota residents who have a disability, or need long-term care services and supports- Assessment must be completed to determine eligibility NOTE: A developmental disability is defined as a severe, chronic disability which originated at birth or during childhood, that is expected to continue indefinitely and restrict the individual's functioning.
