Targeted case management service through Rule 79 that provides services including:
- Development of an individual goal plan
- Assistance with housing
- Daily living assistance
- Chemical and physical health management
- Financial management
- Connection to educational and employment opportunities
- Referrals to resources in the community
- Reassessment of goals every six months to ensure needs are being consistently meet
