Provides information on available resources and how to access the following: - Advocacy - Assistance accessing resources and programs, including care coordination for UCare seniors and UCare Connect Special Needs Basic Care (CSNBC) - Information about area resources for seniors, including financial assistance programs, Medicare program education, and Personal Care Assistant (PCA) - Provide online support group information and resources for individuals living with Alzheimer's or other dementias and caregivers
