- Open to all who need assistance with health care coverage and resources for affordable health care - Eligibility requirements vary by programs

Required documents

- Date of birth for everyone in household - Driver's license or photo ID - Pay stubs from the past 30 days that show income for all family members who are employed, if self employed have most recent tax return documentation of all other income - Social Security number for each person applying non-citizens need permanent resident card or other immigration documents

Services may include assistance with: - Applying for private health insurance plans - Dental care insurance and finding low-cost dental care services - Emergency insurance - Hospital financial aid applications - Renewing health insurance - Referrals to community and social services
