- Pay stubs from the past 30 days that show income for all family members who are employed, if self employed have most recent tax return
- Social Security number for each person applying

Provides in person and phone consultations to help individuals and families apply for health care programs, including MinnesotaCare, Medical Assistance (MA), and MNsure. Services may include assistance with:
- Applying for private health insurance plans
- Dental care insurance and finding low-cost dental care services
- Emergency insurance
- Hospital financial aid applications
- Renewing health insurance
- Referrals to community and social services
