Open to all individuals living with, affected by, or at risk for HIV/AIDS; excludes Anoka, Carver, Hennepin, Chisago, Dakota, Isanti, Ramsey, Scott, Sherburne, Washington, and Wright counties

Required documents

Proof of HIV status proof of income eligibility (400% of the Federal poverty level)

Description

Assists people living with HIV with transportation support through a network of staff and volunteers. Offers mileage reimbursement, bus passes, or taxi vouchers for eligible clients to help them get to and from medical appointments and other supportive service visits when no other transportation resources are available.
