Connects older adults and their family caregivers to resources, support, and enrichment opportunities to enhance quality of life and promote independence. Services include: - Connections to community resources to promote wellness, engagement, and aging in place - Memory loss enrichment programming for adults living with early- to mid-stage memory changes - Partnerships with congregations, such as the Care Ministry Network, to support ministries serving older adults - Support for family caregivers, including coaching, support groups, educational classes, and access to community resources The program emphasizes individualized support, education, and community engagement to help older adults and their caregivers thrive.
