Eligibility

Individuals interested in donating eye tissue in Minnesota; North Dakota; Wisconsin counties of Pierce, Polk, and Saint Croix

Required documents

Donor coordinators will work with living individuals, or family members on behalf of an individual at time of death, to obtain required documents

Application process

To donate, individuals must document the decision in one of the following ways:
  • Create a healthcare directive
  • Mark "donor" on their driver's license
  • Carry a signed donor card
Notification of death and referral for potential donation opportunities must be done as soon as possible following death. Referrals accepted from health care facilities, hospitals, hospices, nursing homes, medical examiners/coroners, law enforcement, funeral directors, and family members
