Application process
To donate, individuals must document the decision in one of the following ways: - Create a healthcare directive - Mark "donor" on their driver's license - Carry a signed donor card - Register online at www.donatelifemn.org Notification of death and referral for potential donation opportunities must be done as soon as possible following death. Referrals accepted from health care facilities, hospitals, hospices, nursing homes, medical examiners/coroners, law enforcement, funeral directors, family members.
