Wellness Classes - Amherst H. Wilder Foundation

Helps older adults manage chronic conditions to improve quality of life by offering: Living Well with Chronic Conditions: A Chronic Disease Self-Management Program provides tools to helps people manage chronic conditions such as diabetes and heart disease through a six-week educational workshop. The workshop meets for two hours each week and is held at a variety of locations. Familiy and friend caregivers are welcome to particpate.

Fee

Sliding fee based on income is used to determine cost of program. Call for fee information. No one is turned away due to financial constraints.

Required documents

During the first class, participants will be asked to complete a form with demographic information and indicate level of income.
