Individuals who are experiencing homelessness or are unhoused and in need of short-term medical care, are residents of Cottage Grove, and are: - Ages 21 and over currently receiving Medical Assistance/Medicaid (MA) - Ages 19 and over currently receiving MinnesotaCare - Needing short-term medical care for less than 60 days
