  • Adults age 18 and older with intellectual disabilities, developmental disabilities, or other health needs who are eligible for the Developmental Disability (DD) Waiver
  • Youth Crisis Stabilization Homes: Children age 4 -18 who are experiencing a mental health crisis

Operates 4 Intermediate Care Facilities (ICF) that house 6- 10 residents and 25 Community Residential Services that house 3 -5 residents across Carver, Hennepin, and Scott counties, with services including:

- Assigned nursing staff at each location

- Recreation department with regularly scheduled programming

- Skills training and social development for adults

Additionally, manages youth crisis stabilization homes in Richfield and Bloomington. The program provides each child with a safe, stable, and nurturing environment, while working with case managers to secure an appropriate long-term home.
