Adult Community Services - DeafBlind Services Minnesota

A community-based program serving adults with significant combined hearing loss and vision loss through a variety of services including: - Independent living skills training and supports via the CADI waiver for deaf, blind/hearing and deafblind adults. Independent living skills training is only available to individuals in the 9-county metro area. - One-to-one Support Service Providers (SSP) to assist deafblind adults with accomplishing the tasks of daily living and community integration. Consumers must be self-determining (able to make their own decisions and direct service providers). - Recreational and leisure activities (various) provided primarily in the Center and some community-based. May be small fee for some activities.

Fee

- Call for fee information; donations accepted; typically paid by county/state - Support Service Providers (SSP) services are free to eligible deafblind consumers and are funded by a Minnesota Department of Human Services grant or county funding. - Independent Living Skills/Supports services are paid via Medicaid Home and Community-based Waivered Services funding: the Community Access for Disability Inclusion (CADI).

Required documents

Proof of Minnesota residency; audiological and ophthalmological reports may be requested. For ILS services, referral from client's CADI Case Manager.

Adults 21 years and older with a significant vision and/or hearing loss or combined hearing loss and vision loss (dependent on service program); must be a resident of Minnesota
