Call; staff will complete an intake interview over the phone to make sure that new patients qualify for services. New patients (or their guardians) will receive a packet in the mail with forms to complete and return. Once the clinic has received the paperwork, staff can schedule an appointment. NOTE: Patients, guardians or group home staff must call the clinic at least 24 hours ahead of time to cancel an appointment. Patients who do not provide proper notice before canceling an appointment may lose the ability to pre-schedule future appointments.

Provides specialized dental services for individuals with complex medical and behavioral needs. The program offers specialized dental care and operates dental clinics in key regions of the state to serve patients whose conditions make it difficult to obtain care from traditional dental providers. Eligible patients include individuals with developmental and intellectual disabilities, severe and persistent mental illnesses, and traumatic brain injuries. The clinics are designed to offer comprehensive, accessible dental care in a supportive and clinically appropriate setting.
