-Patients of HealthPartners Clinics; individuals must first apply for and use all government program assistance and if denied, they may still qualify for the Health Partners Clinics Financial Assistance Program; statewide

last Federal Tax Return or Income Earnings Statement from Social Security Administration if you did not file taxes last year
Copies of your last 60 days of pay stubs
Latest financial statement supporting liquid asset holdings and Social Security letter or unemployment benefit letter, if applicable

Application process
Visit website for application/registration form. After the application is received, a letter is sent to the applicant within 10-14 days by mail notifying them if they qualify for the program or not. If they qualify, the amount the bill is discounted is included in the letter. The program qualification lasts three months from the approval date. Individuals need to apply again after three months to see if they qualify for the program.
