MnCHOICES - Nobles County Community Services

A comprehensive web-based application that integrates assessment and support planning for people who need long-term services and supports in Minnesota. The assessment takes a person-centered approach to ensure that services meet each individual's strengths, goals, preferences, and assessed needs. The assessment replaces the following assessment tools: - Developmental Disability Screening ?- Long-term Care Consultation Assessment ?- Personal Care Assistance Assessment ?- Home Care Nursing Assessment (to be included in a future enhancement) The assessment determines eligibility for publicly funded programs and services including: ?- Alternative Care Program ?- Brain Injury (BI) Waiver ?- Community Access for Disability Inclusion (CADI) Waiver ?- Community Alternative Care (CAC) Waiver ?- Developmental Disability Waiver ?- Elderly Waiver ?- Case management for people with developmental disabilities - Semi-Independent Living Services
