Eligibility

In-home and community-based services available to youth who are in foster care and are residents of Anoka, Carver, Dakota, Douglas, Hennepin, Ramsey, Scott, Sherburne, Washington and Wright counties. Youth receiving in-home counseling must be on Medical Assistance and be residents of Anoka, Carver, Dakota, Douglas, Hennepin, Ramsey, Scott, Sherburne, Washington and Wright counties in Minnesota.
