Refugee and Immigrant Mental Health and Social Adjustment Services - Community University Health Care Center

Offers Adult Rehabilitative Mental Health Services (ARMHS) for Southeast Asians, Latinos and Somalis suffering from severe Post Traumatic Stress Disorder, depression or other mental illnesses. Services include: - Advocacy for daily living problems - Adult mental health case management (Rule 79) - Crisis intervention - Psychiatric services including diagnostic assessments and treatment - Service to refugee and immigrant victims of sexual assault, domestic violence, and child abuse - Supportive counseling for individuals and families - Support groups

Application process

Call; email - Arrive 20 minutes before first appointment with ID and insurance or household income. - In order to be eligible for sliding scale fee, must be either enrolled or in Medical Assistance or MNCARE or have applied within the last 30 days. If Medical Assistance has been denied within the last six months, may show denial and qualify for sliding fee scale based on income and household size.

Fee

- Call for fee information; paid by insurance; sliding scale - Patients who are uninsured and reside in Hennepin County may be eligible for discounted services. Clinic is able to set up a payment plan if requested. A 5% discount is given if the bill is paid in full on same day as visit. (Some restrictions apply to certain services.)

Some form of ID and if insured, copy of insurance card, medical assistance card, medicare card, State ID; Proof of income in the form of check stubs, tax returns or letter from employer to apply for sliding scale fee

Eligibility

- Residents of Anoka, Hennepin, and Ramsey counties; priority is given to Southeast Asians, Latinos and Somalis suffering from severe Post Traumatic Stress Disorder, depression or other mental illnesses - Must have completed a mental Health Diagnostic assessment within the last 12 months
