Reference from outreach worker, mental/medical health worker, or shelter
24 hours/7 days a week

A referral is required through the Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc).

Provides permanent supportive housing for those experiencing long-term homelessness, living with a mental illness, physical disability, and/or substance use disorder. Supports more than 100 residents in affordable rental housing, on-site and scattered around Hennepin County.
