8 am - 4 pm Monday - Friday

- Call to schedule an assessment, which can be scheduled within a couple of days. Nurses do home visit assessments, involving the client and family members, including reviews of medication and ability to perform daily activities. Services can be provided within a week of assessment, depending on need. - Transportation requests must be made at least 2 weeks in advance

Depend on services needed, call for information. Transportation provided without charge by volunteers.

Description

Program provides or arranges whatever support an elder neighbor needs to continue living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers, and/or paid nursing staff. Services include: - Case management - Door-to-door transportation provided by volunteers. Vehicles are not wheelchair accessible. - Health assessment - Home health aides - Information and referral, including to county services - Long term illness management ?- Medication management - Phone visits - Service coordination - Skilled nursing care - Volunteer errands and visits - Volunteer services
