Can assist with short-term goals and long-term goals, but not intended to assist with immediate/urgent needs. Health Coaching: Personal, confidential, one-on-one appointments to work on health goals of various natures - examples include: tobacco secession, nutritious eating, weight loss, improved exercise, and dealing with chronic conditions Health Workshops: Once a month group sessions on health topics. Examples include the power of sugar, healthy eating on a budget, ending tobacco use, and managing pain without drugs Community Health: Community Health Worker can meet with clients to help them navigate life and interface issues with health and human/social service systems. Designed to be flexible and meet people where they are at to help them overcome all sorts of issues that ultimately impact health.
