Metro Intensive Treatment Team - MITT - Mental Health Resources

Provides intensive case management to adults with mental illness who need extra support when their symptoms are at an elevated level. The person receiving services and the team mutually agree upon individualized treatment goals and interventions, with short-term strategic interventions intended to stabilize symptoms and prevent or shorten hospital stays, avoid commitment to long-term hospitalization, and prevent the loss of employment, housing, and personal relationships. Services include: - Assessment of mental health, chemical health, and functional status - Coordination of referrals and community resources - Coordination with the person's providers and support systems - Crisis intervention - Family support and education - Monitoring physical and mental health status - Skill building in the areas of medication and symptom management - Relapse prevention
