If applicable, case manager name and contact information 3. Any of the following that is available to make a proper evaluation: H&P, current diagnostic assessment, discharge summary (if available), or any paperwork that gives a most recent history. 4. Financial Information: Income source for treatment: (MA#, insurance or self-pay?) Income source for Cost of Care: (SSI, RSDI; or combination of SSI/RSDI) 5. Provides intensive mental health and dual diagnosis treatment for an adult co-ed population of 14 residents - Curriculum is provided through group and individual processes in cooperation with the individual focusing on learning the skills to manage one's disability and pursuing personal goals - Additional groups include socialization, recreation, leisure time activities, and independent living skills training
