Fax, mail, or email the following information: Once received, a determination concerning the admission will be made within 72 hours. 1. Name, date of birth, and social security number of recipient 2. If applicable, case manager name and contact information 3. Any of the following that is available to make a proper evaluation: H&P, current diagnostic assessment, discharge summary (if available), or any paperwork that gives a most recent history. 4. Financial Information: Income source for treatment: (MA#, insurance or self-pay?) Income source for Cost of Care: (SSI, RSDI; or combination of SSI/RSDI) 5. Current medication list, (MAR preferred) and current med orders - (to expedite prescribing medication, if admitted) Refer to website for more information

- Primarily serves residents of Saint Louis County, although referrals are accepted from all over the state of Minnesota - Serves adults with chemical health issues who are in need of intensive residential treatment
