Description

Provides care management and coordination for older adults and people with chronic or lengthy illness. Coordinators assist caregivers by answering common questions and concerns. Once a care plan has been decided, coordinators may help with implementing the plan, processing paperwork, and participating in care conferences at nursing homes/assisted living facilities.
