Housing Support - White Earth Nation

Provides room and board in eligible settings for low-income older adults and adults with disabilities with the goal of preventing institutionalization and homelessness. Individuals who do not qualify for home and community-based waiver programs may be able to receive supplemental service to provide other services, including but not limited to: - Arranging for appointments, meetings, and medical and social services - Assistance with transportation - Medication reminders

Eligibility

Low income older adults or adults aged 18 years and older who are living with a disability, are Native American enrolled as a White Earth Nation band member, who have limited to no income or resources, and are residents of Becker, Clearwater, or Mahnomen County
