- Families and individuals must meet income eligibility requirements being at or below 200% of Federal Poverty Income Guidelines - They must provide proof of their emergency, such as an eviction notice - And must have applied for emergency assistance through Hennepin County and received a denial letter - WeCAN areas served: Greenfield, Independence, Loretto, Maple Plain, Minnetonka Beach, Minnetrista, Mound, western Orono, Rockford, Spring Park, Saint Bonifacius, and Tonka Bay
- Application/registration form eviction notice mail with current address picture ID proof of address proof of household income Social Security card/number - Families and individuals who meet income eligibility requirements, must provide proof of emergency, county or other agency ineligibility or denial letter
