Access to well-child checkups for children and teens up to age 21, including young parents, who are on Medical Assistance or MinnesotaCare. Services include: - Assistance in locating a medical or dental clinic - Assistance in making an appointment - Arranging for transportation to these appointments - Arranging for interpreters if needed at appointments - Connecting families to community resources Appointments may include: - A physical exam - Baby Tracks program to help parents and guardians stay up to date with infant and childhood immunizations - Developmental, growth, and mental health screenings - Hearing and vision checks - Lab tests - Regular dental appointments - Information about physical health, mental health, healthy habits, and nutrition Visit website to find a local Child and Teen Checkup Clinic.
