Description
Evaluation, care planning, service coordination, placement, and ongoing monitoring of care for people who have expressed a desire to go home after a nursing home stay of less than 45 consecutive days. Service is available statewide through MN Help Community Living Specialists housed in each Area Agency on Aging.
