Open to all uniformed service members, national guard/reserve members, survivors, former spouses, Medal of Honor recipients, others registered in the Defense Enrollment Eligibility Reporting System (DEERS), and their families

Enrollment forms military discharge papers (DD Form 214) registration in DEERS valid uniformed services identification card
