Including things like community engagement opportunities, group health and wellness programs etc. Services may vary by site.

Application process

Call, email or visit website for listing of housing communities and for an application. Most have waiting lists. - Supportive Housing: A referral is required through the Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc). Individuals currently staying with friends or family, or who are in treatment will not be able to access Coordinated Entry.

Application; birth certificate; picture ID; proof of household income; health insurance; Section 8 vouchers; Social Security card; proof of citizenship including passport
