NOTE: Application is closed for this program. Funds may be available to prevent homelessness. Single adults and families who are past due on their housing expenses and need financial assistance to cover a damage deposit, first month's rent, past due lot rent, at risk of being disconnected from their utilities, or keep their current housing can apply. ?NOTE: Single adults and families must get a pre-screen through United Way 211 to access this program. The pre-screen information is provided to a Washington County FHPAP provider.

Application process

Call United Way 211 for a pre-screen to access this program. The pre-screen information is provided to Valley Outreach. Valley Outreach will make an in-person appointment with the applicant after receiving prescreen from 211.

Required documents

- Applicants must provide proof of residency. Documents may include: Rental lease, shelter form (from landlord) and/or utility bill. - Applicants must provide proof of total household income for all working adults for the past 30-60 days. Documents may include: Benefit statements, pay stubs, etc. - If a household is requesting rental assistance, they must include full contact information for their landlord so required documentation can be solicited from the landlord by Valley Outreach - If a household is requesting financial assistance to pay past due utilities, they must include their history of past due balance and their most recent statement - Supporting documents for the housing crisis. Examples include: Car repair bill, medical documents, verification of income or job loss, etc.

Eligibility

Single adults and families who meet all requirements and are residents of Washington County: - Homeless or at imminent risk of homelessness - In need of services and/or financial assistance due to a housing crisis - Meet income at or below 200% of the Federal Poverty Guideline
