- Submit the financial assistance application form in person, or by mail (Aspirus St. Luke's Hospital, 915 East First Street, First Floor, Duluth, MN 55805)
Fees
Based on a sliding scale:
- 100% discount if household income is less than 100% of Federal Poverty Guidelines (FPG)
- 90% discount if household income is between 101% and 150% of FPG
- 75% discount if household income is between 151% and 200% of FPG
- Greater of amounts generally billed or 50% discount if household income is between 201% and 300% of FPG

Financial counselors can help individuals understand patients' responsibilities and may offer no-interest payment plans. Other options may include:
- Discount Program: Individuals who are uninsured or underinsured and have incomes less than 300% of the Federal Poverty Guideline may be eligible for help to pay their medical bills
