Caregiver Support: Caregiver counselors available to provide support for caregivers to continue caring for a loved one and care for themselves. Counselors assist with making decisions and problem-solving related to caregiving. This includes individual or family counseling, support groups, and caregiver training and education. Services including:
  • Assessing assistance and referrals
  • Assisting with the coordination and planning for supportive services
  • Decision making by providing support and expertise
  • Developing an informal caregiver support network
  • Developing individualized plans for caregiving
  • Exploring personal lifestyle and the impact of caregiving on health status, relationships, and finances
  • Identifying your needs and preferences
  • Problem-solving
  • Self-advocacy
Resources for Enhancing Alzheimer's Caregiver Health (REACH) - service focused on those who care for someone with Alzheimer's disease or a related dementia. REACH is delivered over a two- to three-month time period, including four core consulting sessions with the caregiver, either face to face or by telephone. Support group for people with a dementia diagnosis and care partners, friends/family 2nd Tuesday of each month Hope United Methodist Church 301 West St Marie Street, Duluth, MN 1:30 pm - 3 pm Training for businesses and caregivers: Dementia Friendly at Work training includes information about dementia and related diseases such as Alzheimer's disease, how to communicate with customers and caregivers, and how to create a friendly physical space within the business. Works with organization to identify and implement changes needed to better assist their customers and their loved ones. Evidence-based programs helps promote self-management and understanding those with chronic health conditions (such as diabetes and chronic pain), fall prevention, and encourage improving the overall state of well-being through education, fitness instruction, and resources. Volunteer training: provides videos to ensure volunteer experience. Videos include:
  • Taking a grocery order
  • Grocery shopping- Ride scheduler (transportation)
