Eligibility

Individuals ages 18 and over who are experiencing substance use issues and are residents of Blue Earth, Brown, Faribault, Le Sueur, Martin, Nicollet, Waseca, and Watonwan counties

Required documents

Comprehensive diagnostic substance use assessment health insurance card medical history physician orders recent progress notes from referring treatment facility referral form social security number

Hours

- Office: 8 am - 5 pm Monday - Thursday 8 am - 1 pm Friday - Morning groups and evening groups

Fees

- Non-residential group treatment $34.00/hour - Non-residential individual treatment $70.00/hour - No charge for continuing care services

Description

Offers an outpatient treatment program for day and evening intensive non-residential programming. Services may include: - Co-occurring behavioral health group - Individual therapy once a week per treatment plan - Individualized treatment services depending on recommendations, the participant's individual progress, and completion of treatment plan goals - Intensive non-residential program which include completion of treatment plan goals and family and specialty groups - Opportunity to participate in a General Education Diploma (GED) program as part of the treatment goal plan
