Social Services: - Community resource referrals for housing, shelter, food shelf, chemical dependency treatment, and emergency and crisis support - Medical insurance application and eligibility assistance - Medical transportation - Medicare questions - Tribal benefit information Community Health Programs: - Breastfeeding Support and Education - Cancer screening education and navigation - Diabetes education, including class and support groups - Enrollment and navigation of Minnesota Department of Health Sage Cancer Screening Program - HIV/Hep C testing and Harm Reduction education and resource navigation - Nutrition counseling - Smoking cessation and navigation

Provides medical, dental, behavioral health, and substance use services designed to meet the needs of the urban American Indian community, but open to all. Serves patients regardless of ability to pay, with a specific focus on providing access to culture as a form of healing, harm reduction services, and whole-person care.
