Description
Provides family home visiting services by public health nurses and family health staff to support families, parents, caregivers, and children. - Home visits for pregnant women to support healthy pregnancy behaviors and preparation for labor and delivery - Home visits for families with newborns and young children, including education on child growth and development, nutrition, parenting, car seat safety, and connection to community resources - Home visiting services for families experiencing parenting stress or crisis; services are supportive and educational and are provided or overseen by public health nurses; families may self-refer - Home visits for children with disabilities or special health care needs, including health education, resources, and referrals Visit frequency is based on family needs and may range from one or two visits to longer-term services (up to three years). Visits may be provided in person or virtually.
