  • Children ages birth - 18 who reside in Kanabec County- Child must have a current Diagnostic Assessment with a current Severe Emotional Disturbance diagnosis- Child's family must request or consent to services

Children's mental health case management services including:- Assist parents in advocating for their child's mental health needs- Assist the family to obtain a current diagnostic assessment, if needed- Be a source of information and support- Complete a functional assessment and service plan- Complete or make referrals for assessments and evaluations- Meet with the family to determine needs- Provide information about and referral to community resources
