Community Based Services are programs designed to assist aging and disabled persons to maintain independent living Services to individuals and families are provided to: - Support and maintain independent living in the community - Assist aging and disabled persons in determining their health care needs - Avoid costly hospitalization and institutionalization - Coordinating home care services - Skilled nursing - Home Health Aide (HHA) - Homemaker - Senior Companion - Respite care - Meals on Wheels (MOW) - Transportation
